We use a company called videodetective for all our streamed video.
Please email me at craig.sullivan@lovefilm.com and I'll pass on the details - I'm pretty confident they cover everything in your spec.

Reviewing your criteria, we can generally support everything you listed via a dedicated server platform and bulk bandwidth.
The throughput, the storage and the support, all easily and well supported. Our support team is top-notch, we work with you more as a partner rather than just another vendor. We'll help to identify the best solution for your requirements and support you as you go forward.
The formats, we can support the Windows, Flash, podcasting, however I need to check on the Real format. We do have some limited support for Real, how much traffic do you anticipate for the Real content, in terms of peak users?
The platform allows you to run any services you like, multiple formats, create channels "on-the-fly", support special events, etc, and we just measure the bandwidth in aggregate and bill via the peak-95th% model, which I can describe if needed.
We officially require up to two weeks from the time we receive a signed order, to provision a server platform. Sometimes they come in faster, but if you choose to go forward please allow two weeks in you planning.
Finally, please send me your response at denis at naxtech dot com so that we can talk directly.
regards,
Denis
